PTSO Student Citizenship Scholarship
Annual Scholarship Awards Night

$750 Awarded by the Freeport High School
Parent-Teacher-Student-Organization (FHS PTSO)
Scholarship Criteria:
● Must be in good academic standing, maintaining an average of a B- or above.
● Demonstration of citizenship through acts that have positively impacted a
population.
● Must be a graduating senior and funds must be used for post-secondary
education.
● Must have attended FHS for a minimum of two years.
● Financial need is NOT necessary to apply for this scholarship.

This checklist is your guide to this application. Any application missing one of these
elements will not be reviewed.
● Completed application form
● Testimonial prepared by the applicant describing how s/he has demonstrated
student citizenship. See page 3 for a specific prompt.
● (2) Letters of recommendation. Please do not submit recommendations that
were used for your college applications. See below.
● Transcript
● Paperclip pages together. Do not staple.
● Present all necessary paperwork to the Guidance office by April 16, 2025.
